cformsII for WordPress
CformsII is a great plugin for WordPress. It easily lets you create contact forms for your entire WordPress site. This is my favorite forms plugin to use in the sidebar of a WordPress site. When using this plugin, taking out all the CSS from the form nicely blends it to your background and makes your sidebar look more interactive. CformsII is made by DeliciousDays a great WordPress plugin developer that has brought other useful plugins like TinyMCE Buttons and SMP.
